What is IRS Form 1095-C?

Most university employees received IRS Form 1095-C in the mail this week. This form must be furnished to you if you were eligible for health benefits in 2015 in order to comply with the Affordable Care Act. This form shows you the months you were eligible for employer-provided health insurance and the lowest monthly premium available to you (not necessarily the premium you paid). Although this is an “important tax document” as noted on the outside of the envelope, this is not a form you must file with your taxes; it is for informational purposes to you only. (But please consult a licensed tax preparer if you have questions.)

Some people were suspicious of this mailing because the return address was from Sandersville and there was an enclosure related to a commercial tax preparation service. Despite these anomalies, this mailing is official communication from the University System of Georgia.
